  • Schedule: Monday Friday (no weekends, holidays, or on-call duties)

Position Summary:
Neighborhood Health Association is seeking a full-time Peer Navigator . The Peer Navigator will serve as a vital member of our integrated care team, providing peer-based support, resource connection, advocacy, and outreach services to individuals navigating substance use recovery, many of whom may also experience justice involvement, homelessness, pregnancy, or co-occurring disorders.This position offers a unique opportunity for individuals with lived experience to serve as a trusted guide and mentor to clients, empowering them on their journey toward recovery and wellness while helping to reduce barriers to care.

Key Responsibilities:

  • Engage with clients to provide outreach, advocacy, and support in coordination with the Recovery Community Resource Center team.
  • Utilize personal lived experience with substance use recovery (preferred) to build trust and rapport with clients.
  • Assist clients in identifying individual needs and coordinate referrals to healthcare providers, treatment programs, housing, employment, transportation, and other community resources.
  • Support clients with navigation through social service systems, particularly for individuals who are justice-involved, homeless, or pregnant.
  • Collaborate closely with care teams, community partners, and external agencies to ensure seamless care coordination.
  • Foster and maintain respectful, culturally competent relationships with individuals from diverse backgrounds.
  • Promote personal responsibility, harm reduction, and engagement in long-term recovery services.
  • Maintain appropriate documentation of services provided and client progress.
  • Participate in trainings, meetings, and ongoing professional development opportunities as required.
  • Perform other duties as assigned.

Position Requirements:

  • Lived experience with substance use recovery strongly preferred.
  • Experience working with individuals impacted by Substance Use Disorder (SUD), co-occurring mental health disorders, homelessness, or involvement with the criminal justice system.
  • Ability to build relationships, offer peer-based support, and serve as a positive role model.
  • Strong communication and interpersonal skills.
  • Knowledge of available community resources and willingness to connect clients to needed services.
  • Sensitivity to cultural, religious, and ethnic diversity.
  • Ability to maintain confidentiality, appropriate boundaries, and professionalism at all times.
