Shift Schedule: 7pm-7:30am (12-hour shift), 3 days a week.


JWCH Mission Statement:
The Mission of JWCH Institute is to improve the health status and well-being of under-served segments of the population of Los Angeles Area through the direct provision or coordination of health care, health education, services, and research.

Recuperative Care (RCC) Program:
The Los Angeles General Recuperative Care Program (LAG-RCC)- is an Interim Housing program that aids homeless individuals in their recovery journey, helping them improve their health conditions and enhance their social and activities of daily living (ADL) skills, all with the ultimate aim of securing stable housing opportunities. Some of these individuals may also grapple with mental health and substance abuse issues, which can occasionally manifest in behaviors such as yelling, using strong language, and displaying anger outbursts. Patients must be homeless, have an acute medical condition, and medically and physically stable. This respite program provides medical care, case management, personal care, 3 meals per day, access to long-term housing resources and placement, and reunification services for clients who have fallen out of touch with their family or support network. Wesley also provides these patients with transportation to and from medical visits and to specialists and other social services agencies in the County.

Position Purpose :

Within the regulatory scope of practice and under the direction of the Program Director, the LVN assists in the provision of direct patient care by performing routine patient care procedures, technical and supportive functions in the recuperative care setting.

Principal Responsibilities:

  1. Performs routine patient care functions as prescribed by established policies and procedures within defined scope of education, training and responsibilities
  2. Uses and practices “basic assessment” (e.g. data collection) and documents data in accordance with the policies and procedures of JWCH and the standards of LVN practice.
  3. Contributes to evaluation of the patient’s individualized interventions related to the care plan and treatment plan.
  4. Performs basic patient care consistent with the policies and procedures of JWCH and the LVN licensure laws of the State of California.
  5. Applies principles of aseptic technique and infection control per policy and procedures.
  6. Consistently uses and reinforces proper body mechanics and appropriate personal protection equipment.
  7. Recognizes the need for emergency care; initiates appropriate procedures within the scope of practice and alerts the Nurse Manager and physician in a timely and appropriate manner.
  8. Conducts patient education and supports patient’s self-management goals according to JWCH policies and procedures.
  9. Delegates selected tasks to a medical assistant, consistent with that individuals job description and the polices and procedures of JWCH.
  10. Assists medical personnel with procedures and/or diagnostic exams.
  11. Keeps patient exam rooms and work areas clean and stocked.
  12. Documents pertinent patient information, procedures performed and patient responses, following established policy and procedures.
  13. Documents in the medical record, all patient care immediately after performing any task in the medical record.
  14. Assists in the maintenance, care and handling of medical equipment
  15. Maintains standards of cleanliness, organization and an adequate level of supplies for the exam rooms or assigned work area.
  16. Demonstrates correct and safe technique in the use of and cleaning of equipment according to specific manufacturers’ instructions and policy and procedure manual.
  17. Responsibility and accountability for adherence to organizational and department standards and policy.
  18. Observes department guidelines concerning absences and reports absences in accordance with departmental procedures.
  19. Self-identifies potentially unsafe systems/processes/situations and takes initiative to report to the supervisor.
  20. Participates in organizational programs/committees as assigned.
  21. Demonstrates effective working relations and works effectively as part of the team to facilitate the organization’s ability to meet goals and objectives.

Requirements:

  1. Must be available to cover both morning and evening shifts as needed.
  2. Current and valid California LVN License.
  3. Current BLS card from the American Heart Association.
  4. At least one-year experience in an ambulatory care setting.
  5. Effective communication skills, both verbal and written, to interact with patients, families, and the healthcare team.
  6. Proficient in performing basic nursing skills, such as taking vital signs, administering medications, and assisting with activities of daily living.
  7. Ability to work collaboratively with other healthcare professionals, including registered nurses, physicians, and support staff.
  8. Experience in working with clients and staff from diverse socio-economic, ethnic and cultural backgrounds.
  9. Current and valid California Driver’s License with current car insurance.
  10. Bilingual (English/Spanish) Preferred.
